Seeking a professional to provide psychosocial assessments and complex discharge planning for two inpatient oncology wards with a large elderly patient population.

About the Role
  • Deliver high-quality psychosocial assessments for oncology inpatients
  • Lead multidisciplinary teams in complex discharge planning
  • Provide emotional support to patients, families, and carers

Key Responsibilities

  • To deliver high-quality care, coordinate with medical professionals to ensure seamless patient transitions
  • To lead multidisciplinary teams in developing tailored care plans
  • To offer ongoing support to patients, families, and carers during challenging times

Requirements

  • A degree in Social Work from a reputable institution
  • Current or eligible AASW registration
  • Demonstrated expertise in discharge planning and assessment skills
  • Fully vaccinated against COVID-19 (including booster)
