About You
We are looking for a qualified and experienced Social Worker with a strong focus on patient-centered care. You will have a Bachelor's degree or entry-level Master's degree in Social Work, accredited by the Australian Association of Social Workers. You will also possess excellent organizational skills, a positive attitude towards work, and experience working with culturally and linguistically diverse communities.
Key Responsibilities
- Problem-solving for individual patients
- Ability to work collaboratively with peers, showing flexibility and adaptability in a multidisciplinary setting
- Discharge planning and coordination
Requirements
- AASW-accredited Bachelor's degree or entry-level Master's degree in Social Work
- Previous work experience in a healthcare setting
- Minimum of seven years' experience as a qualified social worker for a Grade 3 Social Work Role
- Good organizational skills and a positive attitude towards work
- Experience working with culturally and linguistically diverse communities
- Able to work autonomously
- A friendly, outgoing, and reliable demeanor
- Well-developed clinical assessment and complex discharge planning skills
- A current Victorian driver's license
