Key Responsibilities- Conduct comprehensive psychosocial assessments and develop individualized care plans
- Provide counseling and support to patients and families, addressing complex social and emotional needs
- Collaborate with healthcare professionals to ensure seamless care transitions and optimal patient outcomes
- Participate in quality improvement initiatives and contribute to the development of evidence-based practice
- Bachelor of Social Work or Master of Social Work Qualifying
- Eligible for ordinary membership of the Australian Association of Social Workers (AASW)
- Current Victorian Driver's Licence
