As a Community Care Coordinator, you will be responsible for developing and implementing individualised, goal-based care plans that meet the unique needs of each client.
About The Role
This role requires strong customer service and communication skills, as well as the ability to manage multiple priorities and work in a dynamic environment.
Key responsibilities include:
- Conducting assessments and consultations with clients in their homes
- Developing and implementing individualised, goal-based care plans
- Coordinating services and ensuring delivery remains within client budgets and program guidelines
- Building strong professional relationships with clients, families, and care teams
- Monitoring client wellbeing, responding to changes in needs, and managing risks
- Maintaining accurate and comprehensive client files

Ideally, You'll Bring
- Registered Nurse (RN) or Enrolled Nurse (EEN) qualifications
- Experience in community care, home-based care, or case management
- Strong organisational and time management skills
- The ability to build strong relationships with clients and colleagues
- A proactive approach to continuous improvement
