Diabetes Education Role

Your primary objective in this position will be to deliver high-quality diabetes education services to clients, as well as educate team members on the delivery of these services and coordinate the service.

You will be responsible for:

  • Providing an efficient service for inpatients and outpatients on both a routine and urgent basis with high-quality clinical outcomes.
  • Notifying healthcare professionals of patients' conditions and their skills in managing their condition.
  • Collaborating closely with dietitians in the care of individuals with diabetes.
  • Reviewing all hospital newly diagnosed diabetics after discharge via telephone.
  • Ensuring that all inpatients' needs are assessed and they are equipped with the necessary equipment for discharge to minimize continued support post discharge.
  • Maintaining accurate records of all patients attended.

You must hold registration with the Australian Health Practitioner Regulatory Agency (AHPRA) and be a Credentialed A.D.E.A. member or working towards it.
