Clinical Care Coordinator Opportunity

We are seeking a skilled and compassionate Clinical Care Coordinator to join our team. As a key member of our multidisciplinary healthcare team, you will play a vital role in coordinating and supporting the delivery of home-based health care to clients across Australia.

Key Responsibilities:

  • Coordinating and managing acute Hospital Care at Home programs for clients discharging early from hospital to home-based care
  • Supporting and communicating with clients remotely to achieve their personal and clinical health goals within their home environment
  • Implementing, monitoring and modifying in-home care plans to meet client needs
  • Loading new cases into the system
  • Performing ad-hoc administrative tasks as needed
  • Managing clinical case escalations and variations to expected clinical progress

Requirements:

  • Qualification as a Registered Nurse with current AHPRA registration
  • Experience working in a multidisciplinary collaborative team
  • Excellent knowledge of hospital discharge planning
  • Tech savvy - this role requires working across multiple systems
  • Ability to work in a dynamic environment with tight timeframes and competing priorities
  • Excellent clinical reasoning and problem-solving skills
  • Excellent verbal and written communications
  • Strong administration and time management skills
