“**Care Coordinator Role**
We have an exciting opportunity for a dedicated professional to join our Aged Community Care Team as a Care Coordinator. This role is perfect for individuals who are passionate about delivering high-quality care and making a real difference in the lives of others.
The Care Coordinator will play a vital role in coordinating and managing care plans for clients under the Support at Home Program.
Key Responsibilities:
- Coordinate all care management activities for clients within the Support at Home Program.
- Conduct initial and ongoing assessments to evaluate client abilities.
- Monitor costs of services and ensure compliance with operational procedures.
- Develop care plans according to client needs.
- Complete documentation to ensure regulatory compliance.
Requirements:
- Cert III qualification in Aged Care.
- Basic First Aid Certificate.
- Solid Case Management experience.
- Experience creating Care Plans.
- Experience in managing service delivery.
