Your Opportunity

The Nurse Practitioner demonstrates advanced clinical skills, knowledge, and high-level capacity to apply evidence-based care for acute patients in Hospital in The Home or residential aged care environments.

They provide effective autonomous and accountable healthcare, incorporating high-level knowledge and skills at an advanced nursing practice level.

This includes assessment and management of clients for safe and optimal care, direct referrals to other healthcare professionals, prescribing medications, and ordering diagnostic investigations.

The Care in the Right Setting program aims to improve access to healthcare and reduce hospital demand by promoting the right care at the right time for the best outcome.

The program includes various services, such as Hospital in The Home, Geriatric Evaluation Management/Rehabilitation – Hospital in The Home, Residential Aged Care Facility Support Service, and more.
