We are seeking a highly skilled and experienced Case Manager to join our team in a specialist role, focused on rehabilitation and community safety.

The successful applicant will be responsible for managing a caseload of complex and high-risk individuals, providing interventions, risk assessments, case planning, and coordination with service providers and care teams.
As a key member of our team, you will work closely with colleagues to achieve best outcomes for clients, while also contributing to the development of our organisational knowledge and practices.
- Manage a caseload of complex and high-risk individuals, conducting risk assessments and developing tailored interventions and case plans.
- Provide expert advice and guidance to colleagues and stakeholders on rehabilitation and community safety strategies.
- Collaborate with internal and external stakeholders to coordinate services and support, ensuring seamless delivery of client needs.
- Contribute to the development of organisational knowledge and practices through research, analysis and evaluation of programs and services.
Key Selection Criteria:
- Experience in effective case management practices, particularly with complex and high-risk clients.
- Excellent communication and interpersonal skills, with ability to build strong relationships with clients, colleagues and stakeholders.
- Proven ability to model pro-social behaviours and lead by example in promoting positive change within communities.
- Passion for supporting clients in the justice system while contributing to community safety.
- Ability to work collaboratively as part of a multidisciplinary team and contribute to the achievement of organisational goals.
