The Opportunity
We are seeking a skilled Care Coordinator to join our Home Health team, which delivers care services to patients in their homes. As a Care Coordinator, you will play a critical role in ensuring that our clients receive comprehensive and coordinated care.
Key Responsibilities
- Client Assessment and Care Planning
You will conduct initial phone assessments, develop care plans, and set goals with clients to ensure they receive the best possible care.
Service CoordinationYou will coordinate in-home services to support the rehabilitation and recovery of clients, including referrals to long-term services.
Stakeholder EngagementYou will build and maintain effective relationships with healthcare professionals, primary and secondary care organizations, and your non-clinical support team.
Progress Monitoring and Care Plan UpdatesYou will monitor patient progress and update care plans as necessary to ensure clients receive the best possible care.
Liaison with Healthcare ProfessionalsYou will liaise with healthcare professionals and specialists to ensure seamless care coordination.
Discharge PlanningYou will develop discharge plans, including referrals to long-term services, to ensure clients receive the best possible care.
Telephone Support and EducationYou will provide telephone support, education, and advice to clients and their families.
About You
We are looking for a skilled and compassionate individual who is passionate about delivering high-quality care. You will have:
- Minimum 2 years' experience as a Registered Nurse
with unrestricted AHPRA registration.
Recent experience in care coordinationis highly desirable.
Strong customer service orientationand relationship management skills.
Excellent clinical reasoning skillsto ensure clients receive the best possible care.
Ability to work in a fast-paced environmentand prioritize tasks effectively.
Proficient computer skillsto enable fast and accurate data entry into patient electronic medical records.
