Job Title: Clinical Care Coordinator

We are seeking a dedicated and experienced clinical care coordinator to join our team. The ideal candidate will provide supervision and guidance to care staff, ensuring a resident-centric approach and high-quality care.

The clinical care coordinator will be responsible for assessing residents' health conditions, monitoring vital signs, and managing medications and treatments. They will also liaise with healthcare professionals to ensure residents receive high-level clinical care.

Requirements:
  • Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) administered through the Australian Health Practitioner Regulation Agency (AHPRA).
  • Recent experience in caring for high-care and dementia residents.
  • Knowledge of wound care, palliative care, behaviour management, clinical assessments, and care plans.
