Community Based Aged Care Nursing Professional

This role is for a skilled Community Aged Care Registered Nurse to join our multidisciplinary team providing high-quality patient care in the comfort of patients' homes. The successful candidate will possess excellent communication and interpersonal skills, with the ability to work autonomously as part of a collaborative team.
Key Responsibilities:
- Conduct comprehensive assessments of patients in their homes, identifying individual needs and developing personalized care plans.
- Deliver evidence-based care interventions, including medication management, wound care, and education on self-care and health promotion.
- Collaborate with other healthcare professionals to ensure seamless transitions between home and hospital settings.
To be successful in this role, you will need:
- A current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse/Midwife.
- Demonstrated high-level written and verbal communication skills, with the ability to communicate effectively with patients, families, and healthcare teams.
- Excellent interpersonal skills, with the ability to build strong relationships with patients, families, and colleagues.
- Ability to work independently and as part of a collaborative team, with flexibility to adapt to changing priorities and demands.
