Home-Based Aged Care Professional

The Transitional Aged Care program is a home-based rehabilitation service for older people discharged from hospital. Our vision is to provide exceptional care and promote healthier lives.
As a Community Aged Care Program Nurse, you will conduct comprehensive assessments in clients' homes and collaborate with the multidisciplinary healthcare team involved in their care. Hospital avoidance is a priority of this program.
You will visit clients at home and utilise our fleet vehicles. The aged care nurse has expertise in community nursing, working within a team that delivers slow-stream therapies and support to older individuals returning home after discharge from hospital.
The aged care nurse provides holistic assessment, care planning, and case management, including referrals, support, and information to clients and their families/carers regarding long-term services for safe and appropriate care.
- Deliver high-quality nursing care in accordance with Australian Nursing and Midwifery Council (ANMC) National Competency Standards competencies, NSW Health policies, and South Eastern Sydney LHD procedures to achieve patient/client health outcomes.
