About the Role

We are seeking a highly motivated and compassionate Support Worker to join our team at Upper South West. As a Support Worker, you will play a vital role in providing personalized support to individuals with disabilities, enabling them to live their best lives.

Key Responsibilities

  • Support individuals with disabilities in their daily living activities, including personal care, meal preparation, and household tasks.
  • Assist with recreational activities, such as swimming, dancing, and horseback riding, to promote socialization and enjoyment.
  • Develop and implement person-centered plans to achieve individual goals and outcomes.
  • Collaborate with customers, families, and other stakeholders to ensure seamless support and care.

Requirements

  • Relevant qualifications, such as Cert III in Individual Support (CHC33015 or CHC33021).
  • Experience working with individuals with disabilities, preferably in a support worker role.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships.
  • Availability to work a minimum of 15-25 hours per week, with flexibility to work varied shifts, including mornings, afternoons, evenings, and weekends.
