About the Role

We are seeking a dedicated care professional to provide high-level support and guidance to our community as an overnight liaison nurse. This is a part-time, fixed-term opportunity until further notice.

As a liaison nurse, you will work from a hub based at Sunshine Hospital, receiving calls from residential aged care facilities, patients, family members, carers, and other community stakeholders. You will provide evidence-based services, advice to consumers, and arrange follow-up support at home from a clinician.

This role involves offering assistance to community nursing teams by managing and allocating workload schedules for the following day to Western Health at Home teams.

Key Responsibilities:
  • Triage incoming calls from community stakeholders and patients.
  • Provide expert advice to help manage conditions in the community.
  • Escalate care when necessary.
  • Support Western Health at Home teams with planning for the next day (schedules, workloads).
  • Liaise with various stakeholders to ensure patients receive comprehensive support.
About You:
  • Be a Registered Nurse, currently registered with AHPRA.
  • Experience working in senior nursing roles with a proven ability to deliver high-quality patient-centred care.
  • Possess the ability to work independently and without supervision.
  • Highly organised and have excellent time management skills.
  • Strong verbal and written communication skills.
  • Technologically capable with experience using Microsoft Excel and Word.
  • Commitment to developing personal education and skills.
