• Friendly and dynamic team **About the Role:** The Critical Care Liaison Nurse (CCLN) is responsible for the comprehensive follow-up, review, and assessment of patients discharged from the ICU; the facilitation of timely admission/re-admission to the ICU, if appropriate; the sharing of critical care skills with ward based multidisciplinary staff and contributing to the development and improvement of critical care services with St Vincent's. * Post Graduate qualification in Intensive Care Nursing* Current experience in the ICU Nurse Liaison/Outreach field (essential requirement for external applicants) or Clinical Nurse Specialist with extensive Team Leader experience (3+ years as CNS or higher classification preferable)* Demonstrates advanced clinical and patient assessment skills* Possesses current ALS2 accreditation* Demonstrates advanced education, leadership and mentoring skills* Knowledge of research principles* Possess high level communication, negotiation, interpersonal skills, diplomacy and ability to effectively engage with other stakeholders* Self-motivated and demonstrates initiative* Strong problem solving ability* Demonstrates a flexible attitude, adapting to the constantly changing demands of the service and performance* Demonstrates tact and confidentiality in dealing with all stakeholders* Demonstrates a commitment to the skill and knowledge development of staff within the hospital* Demonstrated knowledge of professional, ethical and legal standards* Previous experience within Critical Care Liaison will be highly regarded* This position is offered on a permanent night duty basis* Current National criminal history check, or willing to obtain**We encourage applications from people of all backgrounds and abilities. Please contact us if you would like to know more.* to review all patients discharged from the Intensive Care Unit* to provide consultation to ward staff regarding treatment and management of patients* to aid timely review and care planning of critically ill patients in the Emergency Department (ED)* to share clinical skills to enhance knowledge of ward staff* to avert re-admissions to the ICU or to facilitate early and appropriate admission* to improve communication between ICU and the acute wards* in conjunction with the ICU Associate Nurse Unit Manager (ANUM), support the operational needs of the ICU, this may include but is not limited to, ICU activity, break support, education, assisting with transport of ICU patients; identifying discharge-ready patients and ensuring that these patients are prepared for discharge, including relevant paperwork/documentation, as required.* Acts as a consultant for nursing staff and members of the multidisciplinary team in critical care nursing activities* Works collaboratively with the ICU multidisciplinary team to provide best possible patient care both within the ICU and across SVHM* Implements a continuous quality improvement programme in which patient care outcomes are regularly evaluated and enhanced, including but not limited to, streamlining the discharge process, ensuring effective handover, reinforcing tracheostomy education, early detection of deteriorating patients, and streamlining Goals of Care discussions for patients* Promotes effective relationships with ICU and ward nursing staff and the broader multidisciplinary teams* Contributes to the development and implementation of strategies that facilitate service efficiency and patient access* Identifies patient care and needs in accordance with critical care services provided by the hospital and following hospital policy, guidelines and procedures* Actively collaborates with the multidisciplinary ICU team in the clinical decision making regarding the discharge of patients from the ICU, including the coordination of discharge planning, follow-up review of patients and consultation with acute ward staff* Actively participates in Tracheostomy Review Service including daily assessment of all patients with a tracheostomy insitu, bi-weekly Hospital Tracheostomy Service ward round and evaluation of service provided* Actively manages the Critical Care Liaison review service including patient and family support, documentation of clinical assessments, referral and communication with home unit staff* Facilitates the admission/re-admission of patients to the ICU, if required, in conjunction with ICU and home unit staff* Responds to all inpatient Medical Emergency Team [MET] and Code Blue calls and supports the coordination and management of the response* In conjunction with the ICU, reviews and implements opportunities for improving services to high risk patients post discharge including, but not limited to, establishing new services and creating educational initiatives.* Demonstrates expert knowledge and skills in the clinical environment* Provides high quality patient care, both within and outside the ICU* Provides timely and effective consultancy and advice including: + Close working relationships with all key stakeholders* Effective referral processes developed with relevant units and teams + Participation in hospital committees and forums as directed by the NUM - ICU* Data related to the Critical Care Liaison Service is collected consistently and in a timely manner, in conjunction with the ICU Research team, provide reports to the Director and NUM - ICU on a monthly basis* Participation in the daily ICU handover for review and planning of care for patients, as appropriate* Collaborates and consults with all members of the ICU multidisciplinary team* Ensures all elements of clinical practice complies with hospital policy, guidelines and procedures and participates in updating of such policies, guidelines and procedures* Provides effective communication both internally and externally to ensure all relevant parties are aware of the role and scope of practice* 100% compliance with hospital clinical competencies achieved annually* Participate in multi-disciplinary meetings, including but not limited to, the Night Hug for ongoing ICU support as required* Provides learning opportunities for all nursing and multidisciplinary staff within the ICU including developing and delivering the Medical Emergency Team (MET) Study Day* Maintains standards of practice excellence in line with evidence, professional standing, legal and ethical responsibilities* Works creatively and effectively in providing educational programmes that meet staff needs and enhance patient outcomes, this includes, but is not limited to, MET study days, tracheostomy education, insitu education sessions* Fosters high standard of educational service delivery based upon collaborative practice in a multidisciplinary environment* Following the ICU strategy and in collaboration with Nurse Unit Manager and Director, participates and actively promotes proactive patient outcome research* Participates in relevant quality improvement actives with the ICU, as directed by ICU NUM* Develops and implements an educational programme in relation to critical care, focussing on the 'care of patients at risk'* Collates data in relation to education programme including evaluation feedback* Supports and implements changes
