About the Opportunity

This role involves working closely with NDIS participants to help them understand and implement their plans, connect with services, and achieve their goals.

Key Responsibilities
  • Coordinate supports or support connection to NDIS participants.
  • Assist participants to understand their plans and navigate the NDIS portal.
  • Coordinate and manage a range of supports tailored to individual needs.
  • Maintain accurate records and provide timely reports to relevant authorities.
  • Build effective relationships with service providers and stakeholders.
Requirements
  • Tertiary qualification in social sciences, health, or related field.
  • Minimum 2 years' experience in case management within the disability or health sector.
  • Strong understanding of the NDIS and community-based supports.
  • Excellent communication, negotiation, and IT skills.
  • Prior experience or demonstrated commitment to working respectfully with diverse clients and communities.
