About the OpportunityThis role involves working closely with NDIS participants to help them understand and implement their plans, connect with services, and achieve their goals.
Key Responsibilities- Coordinate supports or support connection to NDIS participants.
- Assist participants to understand their plans and navigate the NDIS portal.
- Coordinate and manage a range of supports tailored to individual needs.
- Maintain accurate records and provide timely reports to relevant authorities.
- Build effective relationships with service providers and stakeholders.
- Tertiary qualification in social sciences, health, or related field.
- Minimum 2 years' experience in case management within the disability or health sector.
- Strong understanding of the NDIS and community-based supports.
- Excellent communication, negotiation, and IT skills.
- Prior experience or demonstrated commitment to working respectfully with diverse clients and communities.
