Job Description

Our organization is dedicated to helping residents live longer, healthier lives. As a Care Manager, you will lead with purpose, supporting your team and making a meaningful difference in the lives of our residents every day.

You will be a key member of our leadership team, supporting the General Manager in managing and leading the delivery of person-centred care for residents, ensuring the highest standard of ongoing assessment, care planning and evaluation and clinical governance.

The daily responsibilities of this role include:

  • Overseeing the daily clinical operations of the care home to ensure our residents receive the best care available.
  • Supporting and leading the care home clinical team, including coaching, mentoring, staff allocations and delegation, performance management and education.
  • Reviewing clinical incidents, complaints and changes, providing solutions to reduce any risks.
  • Leading compliance with AN-ACC assessments.
  • Managing person-centred care planning and evaluation for residents, including case conferencing with families and carers.
  • Fostering a strong team environment.
  • Preparing and conducting clinical governance audits.
  • Providing insights and innovative solutions for continuous clinical improvement.
  • Overseeing medication management in accordance with resident needs and legislative requirements.
  • Liaising with allied health, medical practitioners and other services to provide optimal resident care.
Required Skills and Qualifications

To succeed in this role, you will need:

  • A Bachelor of Nursing with current APHRA registration.
  • Post-graduate qualification in aged care highly regarded.
  • Strong and effective leadership, communication and interpersonal skills.
  • A person-centred care mindset.
  • A passion for nurturing a strong team environment.
  • Ingrained passion and empathy for our residents.
  • Proven track record in delivering quality clinical care within an aged care or acute setting.
  • Excellent clinical assessment and triaging skills as well as strong care planning capabilities.
  • Proven and demonstrated experience in fostering a reporting culture balanced with your effective investigative skills.
  • An understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes.
