As a Care Coordinator, you will play a vital role in connecting individuals with resources that can change their lives.

Key Responsibilities:
  1. Forge strong relationships with local service providers to ensure seamless access to health services.
  2. Assess client needs and develop tailored healthcare plans that promote self-management of health conditions.
  3. Support clients within the healthcare system and connect them to vital primary care services.
  4. Collaborate with community organisations to identify opportunities for social connection and participation.
  5. Track client journeys and adjust care plans as needed.
Requirements:
  • Registration as an Enrolled or Registered nurse with the Nursing and Midwifery Board of Australia (NMBA) and/or qualified Aboriginal Health Worker.
  • Excellent communication skills to engage effectively with diverse communities.
  • Demonstrated ability to conduct comprehensive assessments, develop care coordination plans, and evaluate outcomes.
  • Knowledge of complexities involved in caring for individuals with chronic conditions, mental health, trauma, and substance abuse concerns.
