Care Coordinator

Job Responsibilities- Provide person-centered care to clients in accordance with their NDIS plans
- Assist clients with daily living activities, such as eating, dressing, bathing, grooming and toileting
- Provide transportation assistance and respite care to clients and their families
- Keep accurate records of clients' care plans and progress
- Communicate effectively with clients, their families and other members of the care team
- Follow health and safety regulations to ensure the safety and well-being of clients
- Attend regular meetings with clients and their families to review and adjust their NDIS plans as needed
- Minimum high school education or equivalent qualification
- At least 1 year experience working with people with disabilities
- Experience with dementia is highly regarded
- Strong understanding of the NDIS and its guidelines
- Excellent communication and interpersonal skills
- Strong problem-solving and decision-making skills
- Experience with CRM software and NDIS portals
- Understanding of health and safety regulations
- Ability to work independently and as part of a team
- Basic first aid and care skills
