Key Job Responsibilities

The Team Leader plays a crucial role in supporting individuals with dementia as they transition from hospital settings to their homes or residential care.

This position involves leading a multidisciplinary team across multiple locations, prioritizing high-quality service delivery for clients and their caregivers.

  • Foster a culture of empowerment and support within the team, ensuring staff well-being and safety are considered.
  • Oversight of individual and team performance to ensure effective case management framework implementation.
  • Collaborate with other Team Leaders to meet broader program needs and contribute to administrative, financial, risk, and quality goals.
  • Provide direct consultation and support to clients as required, ensuring timely referral allocation to appropriate staff members.
What We Need From You
  • A commitment to align with our organization's mission and values.
  • Tertiary qualifications in Nursing/Allied Health or related field.
  • Demonstrated experience and knowledge in supporting older people and carers with dementia, emphasizing psycho-social approaches to case management.
  • Ability to manage and support teams of skilled professionals across multiple sites and locations.
