We are seeking a passionate professional to empower people living with disabilities to build skills, connect with their community, and achieve their goals under the NDIS.

Job Overview

This role involves working directly with clients and families, coordinating services that promote independence, inclusion, and choice — delivered at home and in the community.

  • Coordinate life transitions and provide support for daily living and life skills
  • Facilitate employment, community access, and independent travel opportunities
  • Develop strong relationships with local services and networks
  • Ensure services align with client goals and needs

Key Responsibilities:

  • Providing individualised support and coordination of services
  • Building strong relationships with clients, families, and community services
  • Collaborating with other professionals to develop and implement client plans
  • Monitoring and reporting on client progress and outcomes

Requirements:

  • Qualifications or experience in Disability, Social Work, Education, or related field
  • Experience supporting adults and/or children with disabilities
  • Strong understanding of person-centred and strengths-based practice
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
