About the role:

We are seeking an experienced healthcare professional to join our team as a Clinical Care Partner.

  • This is a unique opportunity to work with complex community clients and oversee their care needs through assessment, planning, and coordination of services.
  • You will be responsible for developing and reviewing care plans in collaboration with the client and multidisciplinary care teams.
  • Your role will also involve managing budgets and facilitating multidisciplinary care activities.
  • As a Clinical Care Partner, you will liaise with clinical teams to monitor and respond to changing client needs.
  • You will also be responsible for continuously monitoring, reviewing, and evaluating care plans, services, and outcomes.

You will spend most of your day on the road, engaging with clients within the community. We encourage our team members to be creative and enjoy themselves while contributing to our success.

Key responsibilities:

To succeed in this role, you will need:

  • Registration as a Registered Nurse or Allied Health professional holding a current AHPRA registration and unlimited work entitlements.
  • Demonstrated understanding and sound knowledge of client-centred, community-based care services delivery.
  • Demonstrated experience and knowledge in Community Funding streams and programs.
  • Excellent communication and clinical problem-solving skills.
  • A current Australian Driver's Licence.
  • A roadworthy and insured vehicle to use for work.
  • Confidence in managing your time effectively, including an ability to work autonomously.
