About the Role

  • We are seeking a highly skilled and compassionate Care Coordinator to join our team.

Responsibilities

  • Establish strong relationships with clients and their families, providing emotional support and guidance.
  • Conduct comprehensive client assessments and develop personalized care plans to ensure optimal health outcomes.
  • Collaborate with home care workers to deliver high-quality clinical care and advice.
  • Regularly liaise with clients and their families, addressing concerns and ensuring service satisfaction.
  • Maintain accurate and up-to-date client records, adhering to policy and legislative frameworks.

Requirements

  • Formal tertiary qualification in nursing or a related field.
  • Current registration as a Registered Nurse with AHPRA.
  • Demonstrated expertise within the aged care sector.
  • National criminal history/police check.
  • Working with Children Check.
  • Valid First Aid and CPR certificate.
