Care Manager

The Care Manager is a key position within Calvary Home Care, responsible for delivering person-centered case management that is enablement-focused. This role requires a strong understanding of Calvary Home Care policies and procedures, as well as the National standards of practice for Care Management and the National Code of Ethics.
Key Responsibilities- Time Management and Efficiency: The Care Manager must be able to manage their time effectively, working within expected time frames to deliver high-quality care services.
- Understanding Aged Care Standards: A strong understanding of the Aged Care Standards is essential for this role, as the Care Manager will be responsible for ensuring that care services are delivered in accordance with these standards.
- Liaising with Stakeholders: The Care Manager will need to liaise with various stakeholders, including internal networks and external partners, to ensure that care services are delivered effectively.
- Client-Centered Approach: The Care Manager must be able to monitor and respond to changes in clients' requirements, using a person-centered approach to deliver care services that meet their individual needs.
- Innovative Problem-Solving: The Care Manager will need to utilize innovative and problem-solving skills to address complex care needs and ensure that clients receive the best possible care.
- Demonstrated Experience: The Care Manager must have demonstrated experience in the provision of case management services, particularly in the aged/community/disability sector.
- Driver's License: A current driver's license is required for this role, as the Care Manager will need to travel to visit clients.
- Computer Skills: The Care Manager must have highly developed proficiency using computers, including the Microsoft Office suite, and be able to learn and adapt new databases.
- Financial Acumen: The Care Manager must have an understanding of the financial principles of individualized client funding and experience in monitoring and reviewing individualized budgets.
- Teamwork and Communication: The Care Manager must be able to work in a team and negotiate with internal and external stakeholders to ensure accountability for service delivery.
- NDIS Clearance: The Care Manager must have NDIS clearance or be willing to obtain it.
- Police Clearance: The Care Manager must have police clearance within the last 6 months or be willing to obtain it.
