Job Role Overview

Your Choice is seeking an experienced Support Coordinator to assist individuals with disabilities in achieving their goals. The ideal candidate will work closely with participants and their support networks to build capacity and coordinate informal, mainstream, and funded supports.

Key Responsibilities:

  • Meet with participants to identify and prioritize individual goals and initiate support requirements
  • Assist participants in understanding how to effectively use their NDIS plans
  • Cochain participants and their informal supports on implementing their NDIS plans
  • Promote independent decision-making and choice and control
  • Source and liaise with relevant service providers
  • Build and maintain proactive client-focused relationships with stakeholders
  • Monitor plan outcomes, budgets, and expenditure
  • Support participants in resolving challenges during plan implementation or crisis situations
  • Maintain accurate records and note-keeping
  • Prepare formal written documents as required

Requirements:

  • Previous experience as a Support Coordinator, case management position, or working in the mental health and disability industry
  • Qualifications in community services, disability, mental health, or allied health (or willingness to obtain)
  • Strong knowledge of the NDIS, including operational guidelines and frameworks
  • Knowledge of relevant service providers and mainstream/community services
  • Excellent computer skills and Microsoft Office programs
  • Effective time management and adaptability
