About the Role:

The Care Consultant will cover a portfolio in the South East region of Sydney, ensuring all prospective referrals are responded to professionally and promptly. You will support the growth of Anglicare at Home by showcasing our point of difference through a tailored approach with clients.

Other key responsibilities will include (but not limited to):

  • Ensuring smooth transition from sales intake to the start of care, establishing trust and credibility with potential new clients and their families.
  • Delivery of client care by comprehensively assessing clients' needs and goals, preparing initial care plans and budgets in partnership with the client and family.
  • Ensuring a comprehensive understanding of Anglicare's strategy, aged care standards, and relevant policies and procedures.
  • Ensuring that all client documentation is completed in a timely and accurate manner.

About You:

We are seeking an outgoing and interpersonal individual with the following attributes:

  • Diploma of Service Coordination, Community Services, Case Management, or similar qualification.
  • Previous experience in the Community Care sector undertaking comprehensive assessment of clients.
  • Exceptional customer service and communication skills.
  • Prior experience using client management technology and systems.

Candidates will also require a driver's license and vehicle, with reimbursements for kilometers traveled.
