Austin Health is the major provider of tertiary health services, health professional education and research across three campuses in Melbourne's northeast and state-wide. We are renowned for our world-class research and specialist work in cancer, liver transplantation, spinal cord injuries, neurology, endocrinology, mental health and rehabilitation.
The Hospital in the Home (HITH) Program enables the shift of traditional acute care from Austin Campuses to patients' own homes without compromising patient care. This improves patient satisfaction, reduces hospitalisation costs and enhances hospital flow.
Our strategic plan for HITH focuses on delivering reliable, safe, patient-centred care, digital transformation and innovation in specialist care.
Hospital in the home offers an alternative to traditional ward-based care by current inpatients at Austin Health, patients in the Emergency department or those referred to HITH as part of a planned admission often via a HITH surgical pathway.
We assess referrals for suitability ensuring individuals are clinically stable, have suitable home supports, their environment is safe for staff attendance and they consent to the program.
The program provides complete care coordination services including personal care assistance, dietary supplements, physiotherapy and occupational therapy in patients' own homes.
Common care types in Austin HITH include management of drains & complex wounds, disconnection of chemotherapy infusions, management of IV antibiotics and other infusions, post-op monitoring and review, telehealth monitoring for various care types.
