
Home Care Case Manager

Join our growing Home Care team as a Home Care Case Manager at Ryman Healthcare. You will manage a caseload of Home Care Package Residents across our villages, focusing on delivering an exceptional end-to-end customer experience and wellbeing for older people receiving Home Care Packages.
Responsibilities- Assess resident care needs, develop care plans, and monitor budgets within allocated funding
- Coordinate with care staff to deliver high-quality home-based care
- Act on new leads and enquiries and ensure smooth service delivery
- Focus on customer satisfaction and maximizing outcomes for residents with Home Care Packages
- Work across office, home, and village-based settings with 1-2 days per week at the Southbank Office
- Case management experience within aged care, or understanding of Consumer Directed Care within Home Care Packages, and knowledge of HCP guidelines and standards
- Tertiary qualification in a health-related field; nursing degree highly desirable
- Warm and friendly approach to build lasting relationships with residents and families
- Excellent organizational, problem-solving, computing, client database management, and time management skills
- Excellent written, verbal, and numerical communication skills
- Current driver's license
