Our multidisciplinary team in the sub-acute rehabilitation and GEM programs is seeking a skilled Care Coordinator. The successful candidate will lead patient-centered assessment and discharge planning, providing effective communication with patients and families.
- Timely discharges require effective coordination.
- Care coordination caseloads must be managed to support clients with complex needs.
- Liaising with community services and stakeholders ensures comprehensive care delivery.
The ideal candidate will have strong leadership skills, with experience in mentoring staff and enhancing care coordination capabilities. They will also contribute to access and flow strategies across the unit.
