About Restorative Care Partners

We are seeking a highly skilled and compassionate individual to join our team as a Restorative Care Partner. This exciting opportunity will see you working with clients to promote optimal health outcomes, independence, and functional recovery.

  • As a Restorative Care Partner, you will be responsible for coordinating and delivering restorative and rehabilitative care services in accordance with client support plans.
  • You will work closely with clients, their families, and other healthcare professionals to ensure that each individual receives the highest level of care and support.
  • This full-time, permanent position is based at our Bella Vista office, with hybrid working arrangements available.
Key Responsibilities:
  • Community engagement and stakeholder partnerships
  • Clinical coordination and restorative care planning
  • Ongoing support, advice, and guidance
  • Quality care processes and risk management
Requirements:
  • Current registration as a Registered Nurse or Allied Health professional
  • Specialist certifications/qualifications in relevant disciplines (highly regarded)
  • Extensive experience in clinical coordination, case management, or restorative care within aged care, disability, or community services
  • Strong understanding of reablement, functional rehabilitation, and person-centred care models
  • Proficiency in risk management, compliance, and quality assurance processes
  • High-level communication, problem-solving, and decision-making abilities
  • Competence in using clinical documentation and care management systems
  • Exceptional customer service, interpersonal, and communication skills
