We are seeking a highly skilled and dedicated Case Manager to join our Youth Services team at Anglicare Victoria. As a Case Manager, you will play a critical role in supporting young people aged 10-18 who are at risk of entering the youth justice system.
Key Responsibilities- Work with young people and their families to develop and implement individualized plans to divert them from the justice system.
- Use a strength-based approach to empower young people and their families to achieve short- and long-term goals.
- Conduct intake, assessment, and case management to identify needs and develop strategies for intervention.
- Develop and maintain relationships with community agencies and services to ensure comprehensive support for young people.
- Provide education and training to young people and their families on topics such as life skills, mental health, and education.
- Relevant degree or diploma in Social Work, Welfare, or equivalent studies.
- Minimum of 2 years case management experience.
- Commitment to culturally competent practice and service delivery.
- Ability to work in a planned, outcomes-driven model.
- Strong report writing and case note skills.
- Experience working with young people who have multiple and complex needs.
- A competitive salary at SCHCADS level 5.
