Job Opportunity: Case Manager

About the Role

We are seeking a highly skilled Case Manager to join our team. As a Case Manager, you will be responsible for providing case management services to individually funded clients in the absence of their existing HCP Case Manager.

You will ensure that the service is consumer directed, responsive to client needs, and provides optimal performance and a high-level client-focused model of care.

Key Responsibilities
  • Assist the HCP Case Manager in providing ongoing case management for individual package clients.
  • Update person-centred plans and goals with the client, taking into consideration their preferences for service delivery.
  • Ensure Package expenditure complies with funding requirements and Home Care Workers follow individual client plans.
  • Refer clients to other services as needed and maintain accurate records on client management systems.
About You
  • Demonstrated experience in case management, including packaged care and/or self-funded models.
  • Tertiary qualifications in Health, Social Work, Community or similar fields.
  • Knowledge of the New Aged Care Quality Standards and Commonwealth Home Support Services.
  • Understanding of the needs of older individuals, people with mental health concerns, and their carers.
