Job Description

As a compassionate professional, you will provide support to individuals, families, and communities facing complex challenges.

You will assess client needs, develop care plans, advocate for access to essential services, and collaborate with multidisciplinary teams to promote independence, dignity, and overall wellbeing.

This role requires strong interpersonal skills, cultural sensitivity, and a trauma-informed approach to care.

Key Responsibilities
  • Conduct psychosocial assessments and develop individualised support and care plans in collaboration with clients.
  • Provide counselling, case management, and advocacy to individuals facing mental health issues, disability, housing instability, family breakdown, or trauma.
  • Connect clients with appropriate community resources, support services, and government programs (e.g., NDIS).
  • Work collaboratively with families, carers, healthcare providers, and allied professionals to ensure integrated and client-focused care.
  • Maintain accurate and timely case notes, records, and reports in line with organisational policies and privacy regulations.
  • Participate in team meetings, supervision, and ongoing professional development.
  • Contribute to program development, service evaluations, and continuous improvement initiatives.
Required Skills and Qualifications
  • Bachelor's or Master's degree in Social Work.
  • Demonstrated experience supporting clients with complex needs, including mental health and/or disability.
  • Understanding of trauma-informed care, person-centred practice, and the NDIS framework.
  • Excellent communication, negotiation, and advocacy skills.
  • Strong organisational skills and the ability to manage a caseload independently.
  • Proficiency in using case management systems and Microsoft Office Suite.
  • Current Working with Children Check, National Police Check, and valid driver's licence.
