Position Details:

Permanent Full Time - 1.0 EFT (or 80 hours per fortnight)

Work schedule includes weekends, Tuesday to Saturday (most weekends required, negotiable)

Location: Cardiac Heart Failure and Cardiology - Alfred Hospital

About the Department:

The Social Work Department focuses on improving quality of life and participation in care. We address psychosocial impacts of illness and hospitalization, supporting patients and families through transitions from hospital to home.

The Role:

">

We seek an experienced Social Worker to provide advanced social work services within our Cardiac Heart Transplant and Cardiac Heart Failure Service. Key responsibilities include comprehensive assessments using the Stanford Integrated Psychosocial Assessment for Transplant (SIPAT), and support within the Trauma stream.

Essential Skills and Qualifications:

To be successful, you will require:

  • Eligibility for membership of the Australian Association of Social Workers">
  • Bachelor of Social Work or recognized equivalent">
  • Effective team collaboration and family engagement skills">
  • Experience or knowledge in Family Violence, Child Protection, NDIS, or related areas">
  • Knowledge of hospital social work legislation and policies">

Desirable:

">

Postgraduate qualifications in Social Work or related fields would be advantageous. Previous hospital experience and proficient IT skills are also desirable.
