Cardiology Social Work Opportunity

About the Role:

This position is responsible for providing advanced social work services to patients and their families within the Alfred Heart Transplant and Cardiac Heart Failure Service.

  • Assessing the psychosocial impacts of illness and hospitalization on patients and their families.
  • Providing casework expertise and consulting with colleagues from other disciplines as required.

Key qualifications include a Bachelor of Social Work or recognised equivalent, eligibility for membership of the Australian Association of Social Workers, and skills in initiative, facilitation, and current legislation and policy in relation to social work in a hospital setting.

Responsibilities:

The successful candidate will be responsible for completing comprehensive pre-transplant assessments using an assessment tool - The Stanford Integrated Psychosocial Assessment for Transplant (SIPAT). They will also provide support to social workers within designated units allocated within the Trauma stream by the Trauma Team Leader.

Requirements:

The ideal candidate will have postgraduate qualifications in social work or a related field, hospital-based work experience, and sound IT skills. Desirable qualifications include a strong understanding of cardiology and psychosocial issues evident in this patient cohort.
