Clinical Care Leader
Description:This is an exciting opportunity to join our team as a Clinical Care Leader. As a key member of our clinical care team, you will be responsible for ensuring that all residents receive the highest quality of clinical care.
Responsibilities:- Lead and manage clinical teams across the facility to provide expert care and support to residents.
- Collaborate with the care manager to develop and implement effective care plans that meet the needs of residents.
- Ensure accurate documentation and submission of required information, including AN-ACC requirements.
- Support the development and implementation of policies, procedures, and IT system requirements within the facility.
- Coordinate assessment, care planning, and review processes to ensure seamless resident care.
- Assist in the admission process for new residents, ensuring optimal care assessments and initial consultations with residents and their families.
- Maintain accurate records and comply with organisational and legislative requirements.
- Manage and document clinical risk within the facility, promoting a culture of continuous improvement.
- Demonstrate empathy and a commitment to improving the quality of care provided to residents.
- Minimum 1 year experience in a leadership position, preferably in aged care.
- Current unrestricted Registered Nurse Div 1 qualification with AHPRA registration.
- Postgraduate qualifications in aged care are highly regarded but not essential.
- Proven knowledge and experience in Aged Care clinical assessments, care plans, AN-ACC compliance, and documentation.
- Engaging style that can influence and develop clinical leadership skills among team members.
- High level of computing skills to maintain accurate documentation and reporting requirements.
- A confident team player with excellent communication and interpersonal skills.
- Ability to manage multiple stakeholders and remain composed under pressure.
