As a dedicated Community Occupational Therapist, you will play a vital role in empowering older adults to live safely and independently at home.
About the Role- This position is part of a dynamic multidisciplinary team within a community service focused on aged care and rehabilitation.
- The team's primary goal is to enable older adults to live independently and thrive in their community through person-centered care.
In this role, you will work closely with consumers, carers, and families to provide assessment, environmental modifications, home modifications, appropriate referral, and discharge planning.
Requirements- You must hold registration with the Occupational Therapy Board of Australia.
- Proven ability to exercise professional judgment commensurate with years of experience and establish effectively collaborative working relationships within a multi-disciplinary team and inter-agency networks.
- Demonstrated well-developed interpersonal, written, and verbal communication skills with the ability to communicate with internal and external stakeholders.
- Demonstrated computer literacy and ability to use health information systems.
- Demonstrated ability to prioritise and manage workload demands, meet deadlines, and complete activities (including ability to work within scope of practice and seek assistance when required).
- Commitment to participation in professional development, quality improvement activities, and workplace education.
Please note that this role requires a minimum C Class Drivers licence.
