We are seeking a dedicated and motivated individual to join our team as a Clinical Care Coordinator. As a key member of our healthcare team, you will play a vital role in ensuring the delivery of exceptional clinical care to our residents.

Key Responsibilities
  • Act as a clinical resource for staff, providing education and support to ensure compliance with standards.
  • Focusing on clinical service delivery, compliance, and audit processes, you will also be responsible for care planning in line with new standards.
  • Taking pride in your clinical documentation management skills, you will play a key role in maintaining accurate and up-to-date records.
Requirements

To be successful in this role, you will possess:

  • A nursing degree and current AHPRA registration.
  • Leadership experience in clinical care.
  • Solid documentation skills and knowledge of AN-ACC.

You will be someone who is empathetic and has previously worked in an aged care home. You will also have a real desire to make a difference in people's lives.
