
Care Manager

Delivering exceptional rehabilitation care to injured workers in local communities across New South Wales, our focus is on family values and personalised support to tailor a return-to-work plan for each individual.
The role involves:
- Care management for injured workers requiring physical and/or psychological support to regain employment
- Taking a holistic approach, establishing relationships with health professionals and collaborating within a multidisciplinary team to support the journey from rehab to healing
- Performing various assessments based on clinical expertise, including functional capacity evaluations, worksite, ergonomic or activities of daily living assessments
- Collaborating with stakeholders, including the injured employee, employer, medical specialists and allied health professionals to develop, implement and monitor rehabilitation plans
- Supporting large government employers and employees through partnerships with key organisations

What will make you successful?
- Current unrestricted registration or eligibility for relevant industry body membership (e.g., AHPRA, ESSA)
- Occupational Rehabilitation experience
- Tertiary qualifications in Occupational Therapy, Physiotherapy, Exercise Physiology or Nursing
- Strong clinical reasoning
- Ability to engage with multidisciplinary teams to achieve common goals
- Passion for helping people rehabilitate
- Demonstrated ability to work autonomously
