About the role: Providing case management within a multidisciplinary review team framework, delivering evidence-based community-focused person-centered care. Delivering individual and group-based treatment, managing clinical loads and collaborating as part of a multidisciplinary treatment process. Assessments, evidence-based treatment and case management services will be provided to adults 18 years and over experiencing psychiatric illnesses. Care will be delivered utilizing the principles of Trauma-Informed Care and Recovery, in accordance with the National Practice Standards for Mental Health Workforce and NSW Health Mental Health Competencies.
