About the Role:
We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our team at Baptist Housing's Oak Bay Campus. As a Community Team Leader, you will play a vital role in providing high-quality care to our residents, working collaboratively with our Registered Nurses (RNs) and other healthcare professionals.
Key Responsibilities:
- Assume the role of Community Team Leader, overseeing medication administration, wound care, and resident assessments.
- Utilize the wellness model of care and service philosophies to improve residents' lives, involving them in their care and maintaining their independence.
- Contribute to comprehensive care planning and evaluate service delivery, adhering to the highest standards of practice and ethics outlined by the BCCNM.
- Collaborate closely with RNs for complex cases and consultations, implementing wellness programs and fostering a positive environment for both teams and residents.
- Maintain meticulous resident charting and information using advanced systems like Point-Click Care and EMAR, ensuring accurate reporting of acute resident information to our RNs.
Requirements:
- Graduation from a recognized post-secondary Licensed Practical Nurse program.
- Current Emergency First Aid Certification.
- Current registration in good standing with the BCCNM.
- A minimum of three years of LPN experience providing care to seniors or an equivalent combination of education, training, and experience.
