Senior Personal Carer

We are seeking a compassionate and dedicated professional to provide quality care and support to our clients. As a Senior Personal Carer, you will have the opportunity to make a meaningful difference in the lives of others by delivering exceptional client-centered care.

You will be responsible for conducting initial client intake and screening assessments, developing and reviewing individualised care plans, and providing quality care based on clients' needs.

The successful candidate will manage client information in CRM & CMS, ensuring all details and case notes are kept up to date. You will also engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team and as required.

About the Role:
  • Conduct initial client intake and screening assessments
  • Develop and review individualised care plans with a client-centered approach
  • Provide quality care based on clients' needs, including personal self-care, general house cleaning, social support, respite, transport and shopping assistance

The role involves spending time on the road completing scheduled visits, interacting one-on-one with clients within their homes, supporting with daily activities, and providing care in accordance with their individualised Care Plan. Additionally, you will support with community-based recreational and social activities, transportation, and assistance with shopping and appointments.

Requirements:
  • Certificate III Aged Care, Individual Support or an equivalent qualification - Essential
  • Current Australian Driver's Licence (Green Provisional minimum) - Essential
  • Roadworthy & insured (Third Party Property minimum) vehicle to use for work - Essential
  • Experience with technology, including smartphones, tablets, laptops etc
  • Confidence to satisfy the relevant probity checks required by legislation or policies
