Physiotherapist - Aged Care Specialist

About the Role:
We are seeking a skilled Physiotherapist to join our team at Concentric Rehab Centre. As a Physiotherapist - Aged Care Specialist, you will be responsible for conducting comprehensive assessments of individuals to identify their functional abilities, limitations, and goals.
Key Responsibilities:
- Develop individualised treatment plans and intervention strategies to maximise independence, functional capacity, and quality of life.
- Provide evidence-based therapeutic interventions and collaborate with clients, their families, and a multidisciplinary team to develop person-centred care plans and ensure holistic support.
- Communicate effectively with stakeholders and deliver individual and group therapy sessions to enhance daily living skills, cognitive abilities, mobility, and self-care.
- Provide education and support to clients, caregivers, and support workers to enhance their understanding of physiotherapy interventions.
- Maintain accurate and up-to-date client records and documentation.
