Therapeutic Case Management
About the Role- This role offers an opportunity to lead and support a clinical team, providing supervision, partnership building, and performance monitoring.
- You will be responsible for managing complex therapeutic cases, conducting assessments, and developing treatment plans tailored to individual and family needs.
- Program referrals will be facilitated through intake consultations, with face-to-face services provided across the region.
- Collaboration with young people, families, and care teams is essential to create and monitor safety action plans and achieve therapeutic goals.
The ideal candidate will have a sound understanding of issues involved in working with families with diverse needs.
Prior postgraduate training in Adolescent Mental Health, EMDR, Family Therapy etc. would be highly desirable.
Key ResponsibilitiesTherapeutic Case ManagementLead and support the clinical team, including:
