Case Manager Job Opportunity

This role will involve delivering high-quality, person-centred services to clients and carers across designated regions.

About the Role
  • Conduct comprehensive assessments and develop tailored care plans
  • Provide advice on service options and coordinate referrals

  • Maintain accurate client records and ensure compliance with funding and legislative requirements

The Case Manager will be responsible for ensuring all services meet funding body and organisational guidelines, applying consumer-directed care principles from initial enquiry through to ongoing support.

Key responsibilities include conducting comprehensive assessments, developing tailored care plans, providing advice on service options, coordinating referrals, empowering clients and carers, maintaining accurate client records, and ensuring compliance with funding and legislative requirements.

The Case Manager will contribute to VMCH's reputation as a leader in aged care services by engaging with the community, providing excellent service delivery, and responding to changing client needs.

Required Skills and Qualifications
  • Tertiary qualification in Health or a related field (nursing or allied health)
  • Experience in case management and care planning
  • Strong understanding of aged care reforms and consumer-directed care
  • Excellent communication, time management, and networking skills
