Job Description:

We are seeking an experienced Registered Nurse to join our team as a Care Coordinator and Team Leader. The successful candidate will be responsible for ensuring the safety and well-being of our clients in their own homes, while also managing a growing team of caregivers and nurses.

This is a full-time role, working 5 days a week with some flexibility and availability to work occasional evenings, weekends, and on-call as required. The successful candidate will provide clinical care to our clients as needed, particularly to cover last-minute vacant visits.

Responsibilities:
  • Design tailored and meaningful client care plans that foster positive relationships between our clients and the care team
  • Meet with potential clients to consult on their options for premium homecare services
  • Oversee client clinical care and maintain meticulous care documentation records
  • Be accountable for maintaining high clinical standards
  • Assist in the performance management and development of a growing team of caregivers and nurses
Requirements:

The successful candidate will have:

  • 10+ years experience as a Registered Nurse
  • Passion for making a difference in the lives of others
  • Previous community care and care team management experience
  • Catheter insertion experience (IDC/SPC) and wound care knowledge an asset
  • A self-led leader with the ability to take responsibility for their behavior and success
