Job Overview:

The position of Community Care Coordinator is a critical role within our organization, requiring a skilled and compassionate Senior Registered Nurse to lead and support our care team in delivering high-quality services to clients in their own homes.

This senior nursing professional will be responsible for designing tailored care plans that foster positive relationships between clients and the care team, meeting with potential clients to consult on their options for premium homecare services, and overseeing client clinical care while maintaining accurate records.

The ideal candidate will possess 10+ years of experience as a Registered Nurse, with a proven track record in community care and care team management. Prior experience with catheter insertion, wound care knowledge, and mentorship/coaching are highly regarded.

Key responsibilities include:

  • Developing and implementing personalized care plans that promote positive relationships between clients and caregivers.
  • Conducting consultations with potential clients to discuss their options for premium homecare services.
  • Supervising client clinical care and maintaining up-to-date documentation records.

The successful candidate will be accountable for maintaining high clinical standards, assisting in the performance management and development of a growing team, and representing the organization's brand in all interactions.

Requirements:
  • AHPRA Registration (Registered Nurse).
  • NDIS Worker Screener (or willing to obtain).
  • Working With Children Check (Employee).
  • Up-to-date CPR & First Aid Certificate.
  • Technical proficiency with mobile apps and G-suite.
