We are seeking a skilled Clinical Health Specialist to join our team at Northwestern Medicine. Responsibilities
- Conduct comprehensive health assessments and develop personalized care plans for patients with complex medical needs.
- Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure seamless patient care.
- Prescribe medications, therapies, and order laboratory tests as needed.
- Educate patients and their families on healthy habits, disease management, and medication adherence.
- Participate in quality improvement initiatives and contribute to the development of evidence-based practice guidelines.
Requirements
- Master's degree from an accredited school of nursing or physician assistant program.
- Current licensure as an APRN or PA in the State of Illinois.
- Certification in area of specialization (e.g., pediatrics, gerontology).
- Minimum 1 year of experience as an APRN or PA in an outpatient setting.
