We are seeking a skilled Clinical Health Specialist to join our team at Northwestern Medicine. This role is ideal for an experienced Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) who is passionate about providing high-quality patient care and contributing to a dynamic healthcare environment.
Responsibilities
- Conduct comprehensive health assessments and develop personalized care plans for patients with complex medical needs.
- Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure seamless patient care.
- Prescribe medications, therapies, and order laboratory tests as needed.
- Educate patients and their families on healthy habits, disease management, and medication adherence.
- Participate in quality improvement initiatives and contribute to the development of evidence-based practice guidelines.
Requirements
- Master's degree from an accredited school of nursing or physician assistant program.
- Current licensure as an APRN or PA in the State of Illinois.
- Certification in area of specialization (e.g., pediatrics, gerontology).
- Minimum 1 year of experience as an APRN or PA in an outpatient setting.
