This is a unique opportunity to work as a Case Manager in our Specialist Brain Injury Rehabilitation Unit at Liverpool Hospital. As a Case Manager, you will be responsible for delivering comprehensive case management services to clients living in the community.
Our team provides essential rehabilitation services to clients with brain injuries. You will work closely with a dedicated multi-disciplinary team to ensure holistic care and provide support to clients and their families.
The role involves providing case management services to clients with brain injuries, collaborating with a multi-disciplinary team to ensure holistic care, and working independently and as part of a team.
Key Responsibilities:
- Delivering case management services to clients with brain injuries

Requirements:
- Degree in health related discipline
- Experience in Case Management of clients with brain injury or clients in a rehabilitation setting
- Excellent time management skills and ability to work independently and as part of a multidisciplinary team
- Demonstrated excellent verbal and written communication skills including interpersonal negotiation and conflict resolution skills, and computer literacy in a range of programs
- Demonstrated experience working with, or understanding of, external funding bodies such as iCare/CTP/workers compensation/NDIS, and their guidelines and processes
- Demonstrated understanding of and/or experience participating in quality improvement activities and implementation of evidence based practice and demonstrated commitment to continuing professional development / education
